Date Change to the Electronic Health Record

1. Request the following documents

Copy of the Vaccination Paper Record / Vaccination Card
Copy of ID of the Vaccinee
Name of the vaccination site

2. Verify the information against EVDS

3. Request the additional documentation from the vaccination site using the Escalation contact list – send the Incident Report template where applicable and the citizen information

4. Once documentation received, send the request to evdsqueries@health.gov.za with:

Summary of update information request
Documentation from the citizen
Documentation from the vaccination site
