Request to change ID TYPE  from Passport Number to ID Number

1. Request the following documents

• Certified copy of Identity Document of the vaccinee
• Certified copy of the passport of vaccinee
• Copy of the Vaccination card

2.  Verify the information against EVDS 

3.  Once documentation received, send the request to evdsqueries@health.gov.za  with:

• Summary of update information request
• Documentation from the citizen
