Request to rectify duplicated registration with ID/Passport

1. Request the following documents

• Copy of passport/ID with the correct information
• Copy of the Vaccination card

2. Verify the information against EVDS 

3. Once documentation received, send the request to evdsqueries@health.gov.za  with:

• Summary of update information request
• Documentation from the citizen
