Request for update to Transgender change of ID

1. Request the following documents

• Copy of old and new versions of ID
• Affidavit from citizen

2. Verify the information against EVDS 

3. Once documentation received, send the request to evdsqueries@health.gov.za  with:

• Summary of update information request
• Documentation from the citizen
